Following the nomination and selection process, we encourage all members of the Department to join us for the Inaugural Department of Medicine Awards Ceremony on Month Date, 2026. Hosted by the DOM Well-Being Council, this special event will celebrate the positive impact our colleagues have on the Department, the School of Medicine, and the University community.
Please submit your nominations for the following categories by 5:00 p.m. on MONTH DATE using the instructions provided.
The Department of Medicine is committed to recognizing the outstanding faculty and staff whose contributions strengthen our community and advance our missions in patient care, education, research, and service. We invite nominations for individuals who have demonstrated meaningful impact in the categories below. Nominations are welcome for faculty and staff in the Department of Medicine across all specialties, divisions, tracks, ranks, and positions. Self-nominations are encouraged.

- Nominations must be submitted through the online nomination form below. The form may be completed multiple times to nominate more than one individual.
- Eligible nominees include all full-time and part-time Department of Medicine faculty and staff. Nominees must be active Department of Medicine employees at both the time of nomination and the time of the awards ceremony.
- Nominations may be submitted by supervisors, faculty, staff, senior administrators, departmental leaders, colleagues, and other individuals who have worked with or observed the nominee’s contributions.
- Individuals may submit nominations for multiple candidates within a category; however, the same nominee may only be nominated for one award category.
- Multiple nominators may support or submit nominations for the same individual.
- Each nomination must identify the appropriate award category and clearly describe the nominee’s contributions, accomplishments, and impact on a department, division, team, program, or broader university initiative.
- Nomination statements should be between 250 and 500 words and provide specific examples that demonstrate why the nominee is deserving of recognition.
